TERMS OF USE & CONFERENCE REQUIREMENTS

BEFORE APPLYING ONLINE

1. OASIS has taken liberty to provide an electronic application that is safe, secure and private. You will be asked to provide very personal information relating to your personhood and health in this application. If you do NOT feel comfortable providing such information over the Internet, do not progress with this application and call OASIS directly at 850-314-0950.  If you decide to proceed with this online application, please be sure to properly safeguard your personal information from other viewers, especially if you are using a public or shared computer. Neither OASIS nor the webmaster of this site will be held responsible for an applicant's failure to safeguard his/her personal information from other people who may see or use his/her computer.

All applicants will be asked if they are HIV positive or HIV negative.  If you are HIV positive and have a case manager, you will be asked to provide information relating to your case manager.  If you do not have a case manager, please indicate accordingly.  Whether you are HIV positive or are HIV negative, OASIS will never disclose any information you provide in this application to any outside organization or individual without your permission.
   

HIV+ (POSITIVE) APPLICANTS

1. All HIV positive applicants will be required to pay a $75.00 registration fee at time of registration.  NO EXCEPTIONS!  Registrations will not be approved until your payment has been successfully processed.  The $75 HIV positive registration fee covers: entry into all presentations and events associated with Positive Living. For the HIV positive applicant, all hotel lodging expenses and meals are covered through the generosity of our sponsors. Your $75 registration fee does NOT cover the cost of transportation to or from the conference nor does it cover any additional lodging expenses such as phone or room services. Since our sponsors are covering lodging expenses for HIV positive persons, OASIS will determine hotel room allocations.  For a detailed explanation of what is covered under the $75.00 HIV positive registration fee, please contact OASIS directly at 850-314-0950.  Since OASIS is offering a reduced registration fee to HIV positive participants, you may be asked to volunteer your services and/or abilities throughout the event.  If asked to volunteer, you must agree to assist.  HIV negative individuals are not eligible for the reduced registration fee of $75.00.
   

HIV- (NEGATIVE) APPLICANTS

1. All HIV negative applicants will be required to pay a $150.00 registration fee at time of registration.  NO EXCEPTIONS!  Registrations will not be approved until your payment has been successfully processed.  The $150 HIV negative registration fee covers: meals, and entry into all presentations and events associated with Positive Living.  This fee does NOT cover the cost of lodging, nor does it cover transportation to or from the conference.  Lodging arrangements, as well as transportation to or from the conference, are the sole responsibility of the individual applying.  As an HIV negative applicant, you are responsible for making your own hotel reservations at the Ramada Plaza Beach Resort or the hotel of your choosing in addition to completing the online application provided on this site.  If you plan to stay at the Ramada, be sure to ask for the conference rate.  If you have questions regarding what is covered under the $150.00 HIV negative registration fee, please contact OASIS directly at 850-314-0950. HIV negative individuals are not eligible for the reduced registration fee of $75.00.
   

ALL APPROVED ATTENDEES

1. All approved attendees (regardless of HIV status) are required to observe all rules and regulations set forth by OASIS and the Ramada Plaza Beach Resort.
